New QR code requirements for proof of vaccination

As you may have heard on the news lately the province is mandating that your proof of vaccination will only be by showing a personalized QR code. This will become effective sometime in January 2022 and will also be required to enter City of Markham facilities for any AGOAC programs. Any current non QR code documents that you currently use either on your phone or a hard copy printout will no longer be accepted.

To get your QR code use the link below by either pasting in your browser or clicking the Attachment

This will bring you to the provincial website. You will need your Green photo Health card. For those without a Green photo Health card there are further instructions on the website.

The process is very simple and should only take a few minutes.

If you already have a QR code then you are okay. If you need to get the QR code do not delay.

When entering a City of Markham facility you can either show a printed copy of the QR code for scanning or you can also show the QR code on your phone.
